A case of systemic sclerosis manifested by heart failure associated with a large pericardial effusion

Abstract

|
Systemic sclerosis is a connective tissue disease characterized clinically by a variable degree of skin and visceral organ involvement. Patients with symptomatic cardiac involvement, due to systemic sclerosis, have a poor prognosis. Among the cardiac complications, secondary cardiac complications due to systemic or pulmonary hypertension are most common in systemic sclerosis. The primary cardiac involvement includes pericarditis, pericardial effusion, myocardial fibrosis, heart failure, conduction disturbances, and arrhythmias. Necropsy studies report pericardial involvement in between 33 and 72% of cases, while symptoms of pericarditis occur in only 7 to 20% of patients. The pericardial effusion usually manifests after the other clinical and serologic features of scleroderma. We report a case of scleroderma that presented with a large pericardial effusion, which antedated the other clinical and serologic features of scleroderma. (Korean J Med 75:S917-S920, 2008)
